Assessment Solutions is an Occupational Therapy Practice with a focus on providing holistic and innovative Occupational Performance Solutions for adults. As work is the primary occupation of most adults, the core assessments and solutions offered evaluate work capacity as well as support occupational performance and optimisation measures. Services are offered to both individual persons and juristic companies, where applicable.
Current service offering includes:
- Functional Capacity Evaluations (for all physical, sensory, pain and psychosocial conditions, including specialised Low Vision FCEs)
- Worksite Evaluations
- Disability Assessments
- Ergonomic Assessments
- Accessibility Assessments / Audits
- Disability awareness training
- Ergonomic training (with consideration of OHS Ergonomic Regulations of 2019)
- Case management for return to work / support at work
- Vocational rehabilitation;
- Low vision rehabilitation and services
- Training (virtual and onsite): Disability Awareness; Incapacity Management; Absence Management; Ergonomics (in accordance with the Occupational Health and Safety Act, Ergonomic Regulations of 2019).
These service offerings are considered highly specialised as they can only be performed by a qualified Occupational Therapist. While these service offerings are often considered to be vital in supporting health and wellness in the workplace, they are as often not a benefit covered by medical aids in South Africa and are therefore commonly invoiced on a fee-for-service basis.
Service offerings will be available either onsite at the client’s workplace or at the Practice in Parklands, Blouberg. The service offerings are expanding throughout the West Coast, extending from Paarden Eiland to Saldanha Bay, and offering a fully mobile service throughout the Western Cape.
The individual behind this business is Ashleigh Streak, a passionate and experienced Occupational Therapist with over fourteen years of practice, having graduated from the University of Cape Town in 2010. Her career has been devoted to adult physical rehabilitation, with a deep commitment to vocational rehabilitation, disability management, and case coordination for individuals recovering from workplace injuries.

Leadership roles in national Occupational Therapy groups such as the Vocational Rehab Interest Group (VRIG) and Occupational Therapy in Occupational Health (OTOH).
Hands-on expertise in managing complex cases from acute rehabilitation through to return-to-work and long-term disability planning.
Accreditation in Functional Capacity Evaluations through Workwell, supporting evidence-based assessments and interventions.
A robust record in corporate consulting, insurance-related assessments, and legislative compliance in occupational health, with an ability to establish strong rapport with clients and organisations.
